If you would like to support the museum by becoming a member, please download, complete and mail this form with your check made payable to the Jefferson Valley Museum.

Walk of Names Form
The Walk of Names board walk was one of the first fundraising efforts to support the continued operation of the museum and it continues to provide important revenue to keep the museum open. Boards are engraved by museum volunteers.

Memorial Form
Memorial gifts to the museum are another important revenue source. For those remembered, a brass plaque with their name will be added to the memorial wall in the museum. Please provide all the information requested on the form so we can notify the relatives of your memorial gift.
MEMORIAL DONATION FORM
MAKE A DONATION from your Required Minimum Distribution of an IRA or donate stock directly to the Museum's Vanguard investment account. Contact the museum for details. Donations are tax deductible. We are a 501C3 organization.
